Introduction:

Welcome to Jemike's Fitness Gym, a Christ-centered establishment dedicated to promoting physical wellness in an environment that reflects Christian values. This Membership Agreement ("Agreement") outlines the terms and conditions between Jemike's Fitness Gym ("Gym") and the undersigned member ("Member") on [Date of Agreement].

Code of Conduct:

1. Respect: Members shall treat all staff, trainers, and fellow members with respect and courtesy. Discrimination, harassment, or disrespectful behavior based on race, gender, religion, age, disability, or any other characteristic will not be tolerated.

2. Equipment and Facilities: Members agree to use the Gym's equipment and facilities responsibly and safely. Any damage caused by willful misuse will be the financial responsibility of the Member.

3. Hygiene: Members shall maintain proper personal hygiene by wearing appropriate workout attire, using a towel, and wiping down equipment after use. Shoes and shirts are required at all times while on the Gym premises.

4. Safety: Members are expected to follow all safety guidelines, instructions, and rules posted within the Gym premises. Failure to adhere to safety protocols may result in the immediate revocation of membership.

5. Inappropriate Behavior: Any form of sexual advances, inappropriate behavior, or actions deemed unacceptable by Gym staff will result in immediate termination of membership.

Waiver of Liability:

1. Assumption of Risk: Member acknowledges that the use of the Gym’s facilities, equipment, and participation in fitness classes or activities involve inherent risks, including but not limited to, the risk of injury, illness, or even death. Member voluntarily assumes all such risks.

2. Release of Liability: Member hereby releases, waives, discharges, and covenants not to sue the Gym, its owners, employees, agents, or representatives from any and all liability, claims, demands, actions, or rights of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by Member, whether caused by the negligence of the Gym or otherwise.

3. Medical Clearance: Member represents that they are physically fit to participate in exercise programs offered by the Gym and have no medical conditions that would prevent safe participation. Member agrees to consult a physician before starting any exercise program if necessary.

Miscellaneous:

1. Photography/Video: Member grants permission for the Gym to use photographs or videos taken during Gym activities for promotional purposes unless otherwise specified in writing.

2. Right to Terminate Membership: The Gym reserves the right to immediately terminate the membership of any Member found violating the Code of Conduct or engaging in behavior deemed unacceptable without providing a refund.

3. No Refund Policy: Member acknowledges and agrees that the Gym has a strict no-refund policy once membership fees are paid.
